I'm literally always saying this. imagine specializing in something as interesting as sleep and the human brain and spending all day every day billing cpaps to insurance and telling all your patients to lose weight and take melatonin. they're like one step up from chiropractors for me
I believe you can do a serum test for narcolepsy as well as genetic testing but iirc the former requires a spinal tap (nasty for the patient, semi risky sometimes, but I think they can look for the orexin and hypocretin levels and also the antibodies that attack the orexin, I'm going from memory RN so this probabky isn't accurate but they do it in the lab studies all the time) and the latter isn't 100% definitive. but it would be a fuckin start. but as that paper excerpt says, if you fall into REM sleep during the day a lot, are sleepy a lot, and have uncontrollable sleep attacks, and it's been going on for 3 months or more, that's enough criteria for a diagnosis! it doesn't need a sleep study, drug trial, waiting period, trial by fire, test of mettle on the tree of woe, etc. just give people fuckin modafinil at the first appointment and work out the details later. what is so hard

bro. U seem smart how tf do I even start treating me/cf in myself idk where to even look :(
hey sorry i’m so late getting back to this i’ve been exhausted lol. and wanted to give you a comprehensive response. The things I find are mostly from research articles. what sucks is because the condition isn’t fully understood there are a lot of relevant articles that use different names- cfs/me, post-viral fatigue syndrome (pvfs), long covid, post acute sequelae of SARS—CoV-2. I try to keep up to date and search these terms on pubmed every now and then. Medscape is a good app to quickly see which drugs are being used in clinical trials but i think it might be restricted to med students/ researchers/ clinicians but theres probably some way around that.
if you cbf with all that and just want recommendations: the worst csf/me symptoms for me are definitely neurocognitive and the biggest helper has been modafinil followed by dexamphetamine. i highly recommend some kind of stimulants if you can get your hands on them. other drugs and supplements that have shown high efficacy in studies:
coenzyme q10: i’ve been taking high doses (~900mg; up to 3000mg is recommended for disorders of mitochondrial dysfunction) and I do notice some physical improvement.
creatine monohydrate: 3-5g a day (may take several months to see effects however it’s cheap)
low dose naltrexone- lots of new interesting studies coming out about this. naltrexone is an opioid receptor antagonist usually prescribed to alcoholics and opioid addicts but also acts as an immunomodulatory agent at low doses. I started 2mg/day as of two days ago.
Zinc- i recommend zinc picolinate for higher bioavailability
B group vitamins- particularly B6, B9 and B12. There are relatively common mutations at the MTHFR gene (that may also contribute to cfs/me) that cause difficulty in metabolising B12 and B9- if you know your MTHFR genotype or experience weird side effects from B vitamins like insomnia or anxiety and paranoia, look into methylated forms (methylcobalamin and 5-methyltetrahydrofolic acid).

Hi there!! I saw a post of yours about narcolepsy and it had me curious as to whether I might have it. I honestly ignorantly thought it was just a collapsing on the spot asleep disorder and didn't even consider looking into it further until now.
I've had sleep problems since I was a kid. Insomnia, excessive day time sleepiness leading me to battle to stay awake during class, takes me hours to get up. Then as I hit my teens I started getting sleep paralysis, night terrors and occasional auditory hallucinations when falling asleep.
This has gotten worse and I'm now in my mid twenties. I was prescribed a mixture of olanzapine and melatonin for my insomnia but it's stopped working again. I never bothered with diagnoses or treatment further than this because I'm on a disability benefit for severe anxiety that stops me working anyway. I've just accepted that I sleep at weird times of the day and since I have no job it hasn't bothered me that much. Except in the last year or two I've found that my daytime sleepiness is getting worse again.
I go shopping with my mum because I don't have a license and we get home in the evening and by then I'm TIRED. I always fall asleep in the back of the car. It's not normal tiredness though, it's an intense "I need to get in bed right now and lay down my head because I am so overwhelmed by dizziness and sleepiness that if anyone tries to talk to me a second longer I'm going to burst into tears". When I do get home I drop everything, can't help unpack the groceries, and just make a beeline to my bed and get in still in my dayclothes. I'm asleep in under a minute.
This happens every single time I go on shopping errands now.
It's not disabling me *particularly* but it is annoying. And it's getting worse. Do you think I should speak to a doctor about this and that it might be a sign of narcolepsy? I'm sorry that this ask is so long and I'm rambling now, but thank you for your time!
hi hey! to my knowledge, all of the things you are describing are definitely symptoms of narcolepsy. and i've even read studies that say narcolepsy often gets worse in people as they age. in your situation, whether or not you want to go to a doctor about it is i think up to you. the diagnostic process can be long and expensive, and theres no surefire way to fully treat it. the options given to me by my doctor were either treating the daytime tiredness with vyvance or modafinil(i have been on both of these, if you want i can totally tell you what they are/were like for me), or treating the insomnia with a Very Highly Controlled substance they only distribute out of one pharmacy in the entire USA, i dont remember what it was called. if any of these sound like they might improve your quality of life, then i say go for it!! but yeah i definitely recommend looking into all these options and if you think it's worth it, talking to a sleep specialist. whatever you decide to do, i wish you luck in all your endeavors!!!

Dopamine: the willpower humor
Serotonin: the happiness humor
Adrenaline/norepinephrine/cortisol: the excitement humor (used in rage, fear, sexual arousal and actual excitement)
Endorphin: the pleasure humor, counterbalances pain
Or, maybe instead of endorphin, we want oxytocin? The snuggly humor. Oxytocin is largely relaxing and is produced by the aftermath of orgasms, breast feeding, snuggling, and petting a cat (or any pet animal). An important component of romantic love but not solely limited to it. Vasopressin, in people with testicles, also exists and serves a similar function. Oxytocin also causes uterine contractions, though, so it may be about snuggling but too much of it causes menstrual cramps.
Cortisol is a stress hormone produced by the adrenal glands. Adrenaline is a stress hormone produced by the adrenal glands. They are different and serve slightly different functions (for instance, adrenaline makes the heart beat faster, cortisol releases more glucose into the bloodstream), but they are both part of the stress response. Norepinephrine, the neurotransmitter that works with these guys, helps the brain to coordinate these energy release responses, so if you take a norepinephrine reuptake inhibitor like Strattera or Wellbutrin, this functions a lot like a stimulant in that it encourages your body to release more energy for your use.
So yeah, we are pretty much about balancing the humors. You never hear about acetylcholine because it doesn't affect mood (I think?... it's been 30 years since I studied this.) Or GABA, because there are relatively few medications that fuck with GABA -- GABA calms you, but it's closer to the metal than most of these guys because it's wired into the engineering for sleep, so it's probably harder to make GABA-affecting meds safe. One example of a GABA affecting med is modafinil, a non-standard stimulant that blocks GABA and thus prevents you from getting sleepy. It also gives you headaches and can make you nauseous as hell, which is why I stopped using it. Gabapentin, a medication that makes GABA more available in the body, is mostly used as an anti-convulsant, can fight pain, and can help with alcohol addiction, but probably makes you sleepy due to GABA being one of the triggers for sleep.
It's all a lot more complex than you usually hear about because damn near everyone is familiar with the mood altering drugs, but not too many people are worried about epilepsy, narcolepsy, or medicating themselves out of alcohol addiction. But it's fascinating shit.
is it just me or does the way people treat like "dopamine" and "serotonin" in modern pop psych context read exactly like balancing the humors
